Care Management was created to assist elders and their families in maintaining, or regaining, independence and dignity through the aging process. S.A.G.E is a firm believer of individualism, independence, choice and having options in life.  As Geriatric Care Managers we assist elders in maintaining the highest level of functioning possible in the least restrictive/most appropriate environment to meet their needs and to attain the highest quality of life.
